Your Privacy Rights
How to ask questions about your personal information and exercise applicable privacy rights.
Openly provides a straightforward process for asking questions about your personal information and exercising applicable privacy rights.
Access
Ask whether Openly maintains personal information or consumer health data about you and request access to covered information.
Correction
Ask us to correct inaccurate personal information where applicable.
Deletion
Ask Openly to delete covered personal information and consumer health data associated with you, subject to limited legal, security, safety, fraud-prevention, recordkeeping, or other permitted exceptions.
Portability
Request certain personal data you previously provided in a portable format where applicable.
Withdraw consent
Withdraw consent for processing that relies on your consent. Withdrawing required core sensitive-data consent may make ordinary personalized Openly features unavailable.
Model-improvement permission
Turn optional internal model-improvement use off at any time. Turning it off does not prevent ordinary Openly use.
Ask about disclosures
Where applicable, request information about certain third parties to which Openly has disclosed personal information or consumer health data.
Appeal a decision
If we deny a privacy request, you may appeal by emailing privacy@chaperone.health with the subject line “Privacy Request Appeal.”
What Happens After You Submit a Request
- We acknowledge and review the request.
- We may ask for information reasonably necessary to verify your identity or authority to act for another person.
- We generally respond to verified requests within 45 days, subject to shorter launch targets and any legally permitted extension.
- If we need a legally permitted extension, we will explain why and provide an updated expected response date.
- We will not unlawfully discriminate against you for exercising an applicable privacy right.
A deletion request does not always require deletion of every record. We may retain limited information when reasonably necessary and legally permitted for security, fraud prevention, evidence of request completion, legal obligations, disputes, safety-related recordkeeping, system integrity, or backups until normal rotation. Information retained under an exception is not used for unrelated purposes.
For optional model-improvement permission, withdrawal prevents future eligible use after the withdrawal takes effect. Finalized appropriately de-identified datasets and completed model versions may no longer be linked to your account and may not be technically reversible.
